Accessing Your Medical Records

About Us > Administrative Information

Patients have a right to view, or ask for a copy of, their own medical records. In some circumstances they may ask for the records off other patients, for instance their children, if they have power of attorney or if they are executors of a deceased patient.

There are strict regulations regarding the security of confidential information and the practice is registered with the Information Commissioner for the purposes of the Data Protection Act and also meets NHS Information Governance Guidelines.

In order to meet these requirements, whilst allowing patient to access their records you should apply in writing using a form from the practice or downloaded here.

You should note that there is a charge for providing copies of patient records. Further paerwork may be necessary if you are asking for records other than your own in order to establish your right to access these records.
